On-line Appointments
This convenient, on-line appointment request allows you to make non-urgent appointments at OMC. Simply fill out the electronic form, below, and submit it to our schedulers. Your request will be processed between 7:30 a.m. and 4:30 p.m., Monday through Friday. The time and day of your appointment will be confirmed by phone or e-mail. To cancel or reschedule your appointment, please call 715-294-2111.


The appointment is scheduled after you are contacted by phone or email to confirm the time and day.
